The hot-flush herb on most labels doesn't beat a placebo.

When researchers pooled the trials, soy extract, black cohosh and red clover, the ones this shelf is built on, mostly didn't separate from a sugar pill for hot flushes. A placebo already cuts flushes by a quarter to a half in these studies, and most of these herbs don't do better than that. Sage is the one women ask us about most, and it gets its own answer further down.

So we started somewhere less crowded: the ingredients that do hold up. Genistein at the dose that worked. Ashwagandha and saffron, which have real trials in menopausal women for sleep and mood. Creatine, which most brands here haven't touched, for muscle and memory. We tell you the number, and we tell you where the number is small.

How to take it: One capsule daily with water. Give it about 8 to 12 weeks, and don't take more than one a day.

Why some familiar names aren't here.

A lot of menopause products lead with black cohosh, red clover, or sage. They carry long histories, and many women feel real comfort from them. We hold ourselves to one rule: an ingredient goes in only when strong modern research shows it helps the exact thing the product is for. Here is where each one falls short, so you can judge our reasoning rather than take our word for it.

For these, that evidence isn't there yet, so we left them out. If it grows, we'll gladly welcome them in.

  • Black cohosh and red cloverBoth have been put through placebo-controlled trials, and neither reliably beat the placebo for hot flushes. Black cohosh also carries a rare liver signal.
  • Sage, the one worth explaining properlySearch for sage and menopause and you will find studies that look encouraging, so this deserves a straight answer rather than a shrug. The study nearly every sage product points to gave sage to everyone who took part, with no placebo group to compare against. That matters more here than almost anywhere else, because a sugar pill on its own cuts hot flushes by a quarter to a half. When everyone in a study improves and nobody was given a placebo, you cannot tell whether the sage did it. Pool the controlled trials that do exist and the result does not reach significance, which means the effect could be real or could be nothing. So our position is not that sage fails. It is that nobody has shown it works yet, and we would rather not charge you for an ingredient on that basis. If a proper placebo-controlled trial changes the picture, we will put it in and say so.
  • MelatoninIt's sold everywhere for sleep, but in trials of menopausal women it didn't improve sleep over placebo.
  • BiotinNo real benefit for hair or skin unless you're deficient, and it can throw off common blood tests, including one for heart attacks.

No. These are food supplements, not hormones and not a replacement for hormone therapy. If your symptoms are severe, HRT is the most effective option and worth discussing with your doctor. These sit alongside that conversation, they don't stand in for it.

Not without your doctor's go-ahead. Cool contains soy genistein, a plant compound with mild estrogen-like activity, so anyone with a history of a hormone-sensitive cancer, or on tamoxifen, should check first. Rest, Strength and Renew don't contain phytoestrogens, but please still tell your doctor what you're taking.

Not overnight. Sleep and mood tend to shift over two to eight weeks, hot flushes and skin over eight to twelve. Muscle and memory build with steady use and exercise. If a product promised you instant relief, that would be the thing to distrust.
